Abstract
Total knee replacement (TKR) surgery is a widely performed procedure for end-stage knee arthritis. The choice of surgical approach plays a pivotal role in determining the clinical and functional outcomes of the procedure. The medial parapatellar approach has long been considered the gold standard, offering reliable exposure and reproducibility. However, the low midvastus approach has gained popularity due to its potential benefits, including reduced disruption of the extensor mechanism, faster postoperative recovery, and improved early range of motion (ROM). This systematic review and meta-analysis aim to provide a comprehensive comparison of these two approaches, focusing on clinical outcomes such as postoperative pain, ROM, quadriceps strength, and functional recovery; radiological outcomes such as component alignment; and complication rates including infection and lateral retinacular release. Randomised controlled trials (RCTs) comparing TKA approaches were searched in 13 electronic databases, major orthopaedics journals, and oral communications, ClinicalTrials.gov. Two reviewers 15 independently selected trials and extracted data. By synthesizing data from 25 studies involving 4,200 patients, this analysis seeks to elucidate the strengths and limitations of each approach, providing evidence-based guidance for surgeons. Results indicate that while both approaches yield comparable long-term outcomes, the low midvastus approach demonstrates significant advantages in early recovery and patient satisfaction. These findings underscore the importance of personalized surgical planning and highlight the need for further research to optimize TKR techniques.
